How International Insurance Direct Billing Works in China

Direct billing is the process where your insurance company pays the hospital directly, so you don't have to pay large sums upfront and wait for reimbursement. In China, this system works well at international hospitals but has some important nuances.

What is Direct Billing?

Instead of the traditional model (you pay → submit claim → wait for reimbursement), direct billing means:

  1. You visit a hospital in your insurer's network
  2. The hospital verifies your coverage directly with the insurer
  3. The insurer pays the hospital
  4. You only pay any co-pay, deductible, or non-covered charges

Guarantee of Payment (GOP)

A Guarantee of Payment (GOP) is a formal commitment from the insurer to the hospital that they will pay for a specific treatment. GOPs are typically required for:

  • Planned inpatient procedures and surgeries
  • Expensive diagnostic tests (MRI, CT scans)
  • Maternity packages
  • Dental procedures (if covered)

For routine outpatient visits, GOPs are usually not needed — the hospital verifies coverage at registration.

Pre-Authorization Process

  1. Contact your insurer before the procedure — provide the treatment plan, estimated cost, and hospital details
  2. Insurer reviews your plan to confirm the treatment is covered
  3. Insurer issues a GOP directly to the hospital
  4. Hospital proceeds with treatment, billing the insurer directly

Timeline: GOP approval usually takes 24–72 hours for non-urgent cases. For emergencies, hospitals can request expedited GOPs (often within hours).

What to Do If Direct Billing Fails

Sometimes things go wrong. Common scenarios and solutions:

  • Hospital can't reach insurer: Provide your insurer's China hotline number. If still stuck, call your insurer yourself.
  • Treatment not covered: Ask for a detailed explanation. If you disagree, file an appeal.
  • Out of network: You may need to pay and claim reimbursement. Check if your plan has out-of-network benefits.
  • System down: Unfortunately common. Pay upfront, keep all documentation, and claim reimbursement.
